VNA Community Healthcare & Hospice has a long history of partnering with other community organizations to achieve specific healthcare goals.

Town population health – As the town visiting nurse association for four communities: Hamden, Madison, North Haven and Woodbridge, we offer health education programs, family caregiver support groups, “Ask the Nurse” Clinics, flu vaccines and fall risk screenings. We work closely with each town’s social service department to also provide home healthcare to underserved or isolated patients. We are proud to continue to provide these varied wellness services thanks to town support and funding.

Shoreline Eldercare Alliance (SEA) – VNA Community Healthcare & Hospice is proud to be the certified home healthcare agency representative to this group of organizations that serve seniors in the central CT shoreline area. SEA provides information, resource referrals, education and quality professional services. http://shorelineeldercarealliance.org/

Shoreline Village CT – VNA Community Healthcare & Hospice is the home healthcare provider for this membership group which helps seniors stay independent in their own homes. They offer a wide range of services including transportation by volunteers, health and educational programming, plus the ability to make new friends and have some fun at parties and gatherings. Members of Shoreline Village CT receive a yearly wellness assessment and counseling session from a VNA Community Healthcare & Hospice nurse. Click here for more information.

Colleges and Universities – VNA Community Healthcare & Hospice offers a variety of placement opportunities for nursing students. Students may shadow home care nursing staff or work on special short term projects.
